CAPTA Nurse

This is an advanced Public Health and specialized care nursing position that is responsible for providing comprehensive nursing care coordination services to pregnant women and their families. Responsible for providing knowledge, encouragement, linkages and support necessary to maximize families’ health, well-being, and self-sufficiency. This position is responsible for striving to assure each participant’s continued involvement in prenatal and child health care as well as other needed community services, through actively engaging the participant and building on the families’ strengths, assets and goals. This position conducts home visits, outreach, and plans and coordinates health care services for identified high risk participants preferably via face-to-face visits to assist with the continuity of care. The CAPTA Nurse is responsible for maintaining the highest standards in clinical nursing practice and adherence to the policies, procedures, guidelines and standards of Healthy Start.

This is a contracted position which ends June 30, 2025. This position is eligible for renewal.
